# Failures, corrections and limits September 29, 2026. These observations remain alongside successful results. * The browser verification CLI could not create its socket directory: access denied. No CLI browser success is claimed. Verification continued through the available Codex browser tool. * The first optional Playwright probe used a Windows path without a file URL; Node rejected the module scheme. Correcting the path exposed a separate browser launch failure: spawn EPERM. Full browser process restart was not executed. `scripts/node-reopen.mjs` provides the executable test for an environment that permits it. Actual tab closure and a newly opened tab were tested instead. * Inspection found Verify could report an older in memory snapshot. It now rereads committed storage, clears an answer when the snapshot changes and reports a conflict without overwriting storage. * Browser testing showed that disabling action buttons lost keyboard focus. The action wrapper now restores focus to the initiating control when focus falls back to the page body. The corrected Preserve control retained a visible solid focus outline in the browser check. * A corrupted record initially displayed the default empty count. The final build instead says stored history has a conflict and has not been discarded. * Cloudflare inspection returned HTTP 401 authentication error before any version preparation or deployment. Wrangler's existing OAuth session could not refresh. Its wrapper also encountered a child process restriction; direct CLI execution reached the authentication check and confirmed the expired login. No deployment was attempted after that failure. * All 28 model, editorial, routing and offline fixture tests passed in the first expanded suite. Nine local browser storage checks passed. These are scoped results, not proof of universal durability, security, accessibility or distributed operation. The browser import file chooser was slow, but the actual downloaded file was ultimately imported and verified in independent origin storage. No faster completion or full process restart is inferred from that result. Production inspection found a preexisting release drift: 57 of 59 responses matched the supplied source release. Live Image Credits additionally retained a historical NASA attribution paragraph, and the service worker had a different revision identifier. The attribution was carried into this source package to preserve deployed content. The initial 57/59 comparison is retained in the release verification folder. Production was not changed.
